Long-Term Rehabilitation Case Management

The Long Term Rehabilitation Case Management (LTRCM) Program helps people with severe physical or sensory disabilities live more independently and improve their quality of life.

LTRCM Specialists connect individuals to the right services, supports, and community resources. They help navigate complex systems and work with individuals to create a personalized plan focused on independence, employment, and self-direction.

Support may include exploring available programs and coordinating long-term care services.

Eligibility

Individual must have a documented disability that includes, but is not limited to, one of the following:

  • Traumatic brain injury
  • Spinal cord injury
  • Cerebral palsy
  • Arthritis
  • Muscular dystrophy
  • Multiple sclerosis
  • Prader-Willi
  • Systemic lupus

In order to receive services, all of the following must apply:

  1. You are a Virginia resident
  2. You have a documented disability as outlined above
  3. Your disability creates a substantial barrier in three or more areas of your life, including:
    • Communication
    • Economic Sufficiency
    • Independent Living
    • Learning
    • Mobility
    • Self-Care
    • Self-Direction
  4. You do not have a case manager/care coordinator from any other source
  5. You need case management to meet your desired life goals and objectives

Cost

Long Term Rehabilitation Case Management Services are provided at no cost to individuals who meet disability and financial eligibility.
