Electronically Submitting Evidence

Medical providers (including hospitals, clinics, doctors, and health information managers), school professionals, attorneys, and claimant representatives can electronically upload records and documentation through the Social Security’s Electronic Records Express system. The evidence that is sent is automatically associated with the claimant’s unique disability claim folder, which will immediately be available to DDS.

Becoming a Consultative Examination Provider

When a decision cannot be made based on evidence received from treating sources, DDS authorizes and funds a consultative examination.

Community-based physicians, psychologists and facilities supply these services.
