Screenings & Assessments

Local departments of social services can conduct long-term care screenings and assessments.

Your local department of social services can help you apply for the following long-term care services:

  • Medicaid Long-term Services and Supports screenings for adults seeking nursing facility placement or home and community based services such as CCC Plus waiver or Program for All Inclusive Care for the Elderly (PACE)
  • Assessments for placement in an assisted living facility through the Auxiliary Grant.

How to Request Services

  1. Contact your LDSS
  2. Request a Medicaid LTSS screening OR an ALF assessment
