Becoming a Provider

Facilities that are interested in accepting Auxiliary Grant payments should contact us via the form linked below or by calling the AG Program Manager at 804-662-7531.

The following settings are eligible to accept Auxiliary Grants:

1. Assisted Living Facilities

   Must be licensed by the Virginia Department of Social Services

2. Adult Foster Care Housing

   Must be certified by the Local Department of Social Services

3. Auxiliary Grant Supportive Housing

   Must be certified through the Department of Behavioral Health and Developmental Services
