The following resources may be used to help guardians better understand their duties and responsibilities:

The forms below must be completed when pursuing the following actions:

• To request a periodic review hearing earlier than scheduled, Form CC-1646 “Petition for Review Hearing”
• To restrict contact with another person in limited circumstances, Form CC-1645 “Notice of Restriction by Guardian

Guardians are required to complete initial and annual reports on Form CC-1644, “Report of Guardian for an Incapacitated Person.” The form was updated on July 1, 2025 to add a statement about the Virginia Guardian Training.

Guardians in Virginia are considered mandated reporters. To report suspected adult abuse, neglect, or exploitation, you must contact Adult Protective Services (APS). There are three ways to make a report:

• Call your local department of social services,
• Call the 24-hour, toll free APS hotline at 888-832-3858, or
• Complete an online report.
