Guardians are required to complete initial and annual reports on Form CC-1644, “Report of Guardian for an Incapacitated Person.” The form was updated on July 1, 2025 to add a statement about the Virginia Guardian Training.

Guardians in Virginia are considered mandated reporters. To report suspected adult abuse, neglect, or exploitation, you must contact Adult Protective Services (APS). There are three ways to make a report:
